In the ever-changing healthcare environment, more emphasis has been placed on care coordination and transitions of care as patients transition from one level of care to another. The BCBSM Integrated Michigan Patient-Centered Alliance in Care Transitions (I-MPACT) is one example of the important work our nursing team does to improve care transitions. I-MPACT is a Collaborative Quality Initiative (CQI). The purpose of I-MPACT is to improve patients’ care transitions from hospital to home and from skilled nursing facility to home.
Working with our hospital partners, our nurses are tasked with developing and implementing innovative approaches for improving care transitions for heart failure and chronic obstructive pulmonary disease patients (COPD). The Physician Alliance partners with Ascension Providence Hospital, Ascension Providence Park Hospital, Ascension St John Hospital, Ascension Macomb-Oakland Hospital, and Harper-Hutzel Hospital to improve care transition for identified heart failure and COPD patients.
